One Heart by HeartWorks is a community built by heart parents, for heart families. United by our journeys through congenital heart disease (CHD), we are here to offer the guidance, understanding, and connection that only comes from walking the same path.

As part of the HeartWorks family, One Heart stands beside patients and families through the many highs and lows of the heart journey. We share our stories, our strength, and most importantly, our hearts.

One Heart Programs

Discharge Bag Program

Returning home after a hospital stay is a significant step — filled with hope, healing, and new challenges. Our Discharge Bag Program helps ease this transition, reminding families that they are not alone on this journey. Each bag includes comforting essentials such as a One Heart by HeartWorks collapsible backpack, our exclusive One Heart Sloth Plushie, and other thoughtful items to support your family’s next chapter at home.

This program reflects HeartWorks’ broader mission: to keep families together, to nurture healing at home, and to fuel the ongoing fight for a cure.

In Our Hearts Box

The loss of a child to congenital heart disease is a sorrow that no family should bear alone. Through our In Our Hearts Box, One Heart by HeartWorks reaches out to grieving families with compassion, remembrance, and love.

Each box is carefully curated with comforting items, designed to offer a small measure of peace and to honor the precious life of the child who will always be remembered. We walk hand-in-hand with grieving families, offering a community of support and the reminder that your child’s memory lives forever in our hearts.

Emergency Closet Program

When life changes in an instant, One Heart is there. Through our Emergency Closet Program, we partner with local hospitals to stock essential items for families admitted unexpectedly. Whether it’s a toothbrush, a phone charger, or a blanket for comfort, our Emergency Closets are ready — because no one should have to face an emergency without the basics, and no family should feel alone during their time of crisis.
