Consent to Use of Likeness: I, the undersigned, grant HeartWorks ("the Organization") the irrevocable and perpetual right to use, reproduce, edit, publish, and distribute any photographs, videos, or other media (collectively referred to as "Media") taken of me, my minor children (if applicable), or property during my participation in activities hosted, sponsored, or associated with HeartWorks.
I understand that these Media may be used for purposes including, but not limited to:
• Social media platforms
• The Organization’s website
• Printed promotional materials
• Online advertising
I acknowledge and agree that:
• I will not receive any form of payment, royalty, or compensation for the use of the Media.
• HeartWorks shall own all rights to the Media and may edit or modify the Media as deemed appropriate.
• The Organization will not use the Media in a way that is intentionally defamatory, exploitative, or harmful.
Release of Liability: I release HeartWorks, its employees, agents, and representatives from any claims, demands, or causes of action related to the use or publication of the Media, including claims for defamation, invasion of privacy, or rights of publicity.
Acknowledgment: By checking the box, I confirm that I am at least 18 years old (or the legal guardian of any minor participants), have read and fully understand this Media Release Waiver, and agree to its terms voluntarily.
